Objective: Describe the behavior in the dispensing of drugs for breast cancer in a cohort of patients during 2020.

Method: An observational descriptive analysis was developed in a cross-sectional way, during 21 months, between January 2020 and September 2021, following patients with dispensing of outpatient drugs for breast cancer, distributed in different areas of the country, using descriptive statistics and analysis of costs of treatments dispensed by therapeutic lines.

Results: The total cohort had 3,076 patients dispensed since January 1, 2020, 22.5% of the patients had up to 4 treatments delivered in the analysis period, the treatment with the highest dispensation was Anastrozole in 29.5%, followed by Letrozole 23.3% used in initial lines of treatment. A probability of death of 0.14 per year was established, associated with non-dispensing for 4 months at the end of the individual follow-up. Regarding the general monthly per capita cost of the patients, it was COP$ 250.202 (USD 62,97); the per capita cost of drugs covered by the basic plan was COP $ 42.788 (USD 10,76) and the per capita cost per month of treatments outside the basic plan was COP $ 207.414. (USD 52,18)

Conclusions: A trend is observed in the use of low-cost medications in the analysis cohort that is maintained during follow-up, however, the cost differential in the lines of treatment analyzed is high, therefore, timely treatment would generate savings associated with use of first-line treatments.
