Study of the Availability of Antihypertensive Therapy During Pregnancy During Wartime in Ukraine

OBJECTIVES: In Ukraine (2020), more than 21,000 cases of hypertensive disorders in pregnant women were diagnosed. As of February 2022, about 265,000 pregnancies were registered, but there is no information on whether all women were healthy and whether they were able to give birth to healthy children. It is known that the war in Ukraine led to an increase in cases of diagnosing arterial hypertension during pregnancy, a perinatal mental disorder, premature birth, stillbirth.

RESULTS: The use of antihypertensive drugs during pregnancy can reduce the clinical burden and increase the chances of having healthy babies. We have researched that as of May 2024, the available range of antihypertensive drugs is methyldopa, nifedipine, metoprolol, esmolol, nicardipine, uradipyl, and absent from the pharmaceutical market of Ukraine: labetalol, hydralazine, acebutolol, mepindolol, oxprenolol. The studied group of antihypertensive drugs is import-dependent, 69.3% of drugs from foreign manufacturers, 30.7% from Ukrainian 6 manufacturers. We determined by the "cost minimization" method that the costs of one month of treatment when using nifedipine of the Ukrainian manufacturer (10 mg, Nо50) compared to the drug of a foreign manufacturer (10 mg No50) are 90% lower; for metoprolol of the Ukrainian manufacturer (100 mg Nо30), costs are 73% lower than for the imported drug (100 mg Nо50).Analysis of determining the socio-economic availability of antihypertensive drugs using the price liquidity ratio was 0.18-0.60, which indicates relatively adequate price competition
CONCLUSIONS: There are no 5 proven effective antihypertensive substances on the pharmaceutical market of Ukraine, therefore there is a need to make them available for therapy. There is a need to ensure, with the help of budget funds, rational therapy of arterial hypertension during pregnancy, because it is known that during the war, all medicines are bought by pregnant women with their own money.
