Mental Health and Sleep Disorders in Pediatric Patients with and without Atopic Dermatitis

OBJECTIVES:

To compare the difference in diagnosis frequencies of mental health and sleep disorders in pediatric patients with and without atopic dermatitis (AD).

METHODS:

A retrospective review was performed on 1,000 patients 18 years of age and under, with 500 of the patients having a diagnosis of AD and the other 500 patients having no such diagnosis. AD was identified using ICD-10 codes L20, L20.8, and L20.9. ICD-10 codes used for depression, suicide ideation, anxiety, and sleep disorders were F32, R45.851, F41, and G47, respectively. Descriptive analyses were used for patients’ demographic information. Chi-squared and Fisher’s exact tests were used to identify the association between the diagnosis of AD and various categorical variables. A p-value of less than 0.05 was considered statistically significant. All statistical analyses were conducted using RStudio version 2021.09.2.

RESULTS:

The AD and non-AD groups were composed of similar proportions of female (50.4% vs 51.8%), non-Hispanic white (50.8% vs 53.8%) and non-Hispanic black (21.2% vs 22.4%). Median age (Q1-Q3) was 8.5 (5.5-12.3) years in the AD group and 9.4 (4.9-14) years in the non-AD group. The most prescribed medications were hydrocortisone (39.2%) and triamcinolone (34.6%). Depression was documented in 2.0% of the AD group and 1.6% of the non-AD group (χ2= 0.057, p-value=0.812). Suicide ideation was documented in 0.6% of the AD group and 0.8% of the non-AD group (p-value=1), while anxiety was documented in 5.0% and 4.4% of the groups (χ2=0.09, p-value=0.765). Sleep disorder was more prevalent in the AD group (7.0%) compared to the non-AD group (3.4%) (χ2= 5.8625, p=0.015).

CONCLUSIONS:

A positive association between sleep disorder and AD was observed while other disorders did not show association. The prevalence of depression, suicidal ideation, anxiety, and sleep disorder was overall low in the study population. Additionally, there was low utilization of AD biologics therapy compared to topical therapy.
