Application Effectiveness of Hospital-Wide Glucose Management in Type 2 Diabetes Inpatients

OBJECTIVES: Perioperative glucose abnormalities can lead to increased rates of mortality, infections, and cardiovascular, as well as prolonged hospital stays in patients. Therefore, our institution has established an hospital-wide glucose management model based on CDSS and CGM. This study systematically evaluated the clinical outcomes of hospital-admitted T2DM patients undergoing integrated hospital-wide glucose management interventions.

METHODS: From May 2023 to May 2024, a total of 254 T2DM inpatients admitted to departments including Endocrinology, Orthopedics, Cardiology, Neurology, and ICU were selected. They were divided into control and intervention groups (127 cases each) based on the hospital-wide glucose management model implemented before and after the application.

RESULTS: After the implementation of the hospital-wide glucose management model, the rate of blood glucose monitoring among admitted patients reached 88.77% (compared to 69.20% before management), effectively identifying the target population for management. Among those requiring glucose management (previously diagnosed with diabetes, FPG ≥7.0 mmol/L, GLU ≥11.1 mmol/L, or HbA1c ≥6.5%), the management rate exceeded 95%. Retrospective analysis showed that the intervention group achieved a glycemic control rate of 47%, an improvement of over 10% compared to 35% in the control group. Additionally, the intervention group had a lower incidence of hospital-acquired hypoglycemia (6.8% vs. 11.3% in the control group). The average length of hospital stay was significantly reduced in the intervention group (7.6 ± 1.9 days vs. 9.2 ± 2.5 days in the control group, p < 0.05).

CONCLUSIONS: The hospital-wide glucose management model effectively improves clinical glycemic control in patients, enhancing rates of blood glucose monitoring, management, and achievement of glycemic targets, while reducing incidences of hypoglycemia and length of hospital stay.
