Criteria for Value Assessment of Medication Adherence-Enhancing Interventions: Preliminary Results of a Systematic Review

OBJECTIVES: Medication non-adherence is prevalent across all clinical conditions and causes major medical and economic challenges. While medication adherence enhancing interventions (MAEIs) have demonstrated proven benefits in improving outcomes, to date there is no comprehensive overview on the assessment of these interventions. This study aimed to fill this gap by systematically collect outcome measures used for the value assessment of MAEIs.

METHODS: Systematic literature review was performed using MEDLINE (via OVID), PsycINFO (via OVID), Scopus, CINAHL (via EBSCO), and Academic Search Complete (via EBSCO). We extracted data on outcome measures reported in relevant clinical trials, prospective cohort studies and economic evaluations published in English language papers between 2018 and 2020. The protocol was registered with the PROSPERO, in accordance to PRISMA guidelines (CRD42021242934).

RESULTS: Title and abstracts of 14,685 records were screened, resulting in 1,782 potentially eligible articles for full-text screening. In total, 308 articles were included in the data extraction. Of the included studies, 91 involved behavioral components, 110 used educational strategies and 107 provided mixed educational and behavioral interventions for improving adherence. The median (IQR) number of outcomes per study used to assess the effectiveness of MAEIs was 3 (2). The identified outcome measures were clustered into 6 categories: medication adherence (measured in 233 studies), patient-reported (n=134), clinical (n=127), resource utilization (n=40), economic (n=19) and other outcomes (n=54).

CONCLUSIONS: While a variety of outcomes were used to assess MAEIs, many were considered secondary outcomes. Besides adherence, clinical and patient-reported outcomes, only a limited number of studies measured the impact of MAEIs on resource utilization and economic outcomes. Further studies would be warranted to select and rank the most relevant outcome measures for the value assessment of MAEIs.
