A Super Intelligent Health Services Treatment Delivery System That Dynamically Adapts to a Person's Evolving Needs and Offers Personalized Treatment With Improved Outcomes for Substance Use Disorder in the US.

OBJECTIVES: 82% of the Substance Use Disorder (SUD) prescribers in the US are in urban areas making access to treatment in rural areas difficult. Moreover, ~30% of the patients discontinue treatment increasing the risk of relapse 10-fold. The objective is to build a digital health services delivery platform powered by behavioral economics, machine learning (ML) and artificial intelligence (AI) to not only solve the problem of access to treatment in rural areas but also improve treatment adherence and outcomes.

METHODS: Our ML and generative AI powered tele-health treatment delivery system will have a licensed physician, a therapist, expert talks, meditation modules and a GenAI powered chatbot trained in behavioral economics based contingency management strategies to ensure treatment adherence, foster healthy behaviors and check for anxiety, depression as well as for harmful behaviors and suicidal ideation. Our technology is being built using multi-modal data pipelines – First, we will be using publicly available longitudinal real-world evidence (RWE) Medical Expenditure Panel Survey (MEPS) data-set that is weighted according to the US census to determine the predictors of relapse in patients using Mixed Logistic Regression model. This will be combined with patient’s electronic health records, data from short form surveys and PROs (DSM-5, PSS-10, AUDIT etc.) which will be periodically collected along with real-time data on heart-rate, physical activity and sleep to monitor the patient in real-time for risk of relapse. The AI chatbot will be trained for harmful behavior checks and utilize “boosts” to incentivize healthy behaviors and treatment adherence.

RESULTS: This Gen AI and ML powered SUD treatment delivery system will improve treatment adherence, reduce relapse related hospitalizations and ER visits, improve quality of life and save healthcare costs.

CONCLUSIONS: This super intelligent health services delivery system will lead to improved treatment adherence and outcomes within SUD patients in rural areas in the US.
